Open the ATHLETE REGISTRATION FORM 2012: Fill out and submit, check the paid Online Box and leave the rest of the information blank. To make online payment click the online registration link: The participant must use or create their own bowl.com account and use their USBC ID#. If someone else is paying for the bowler's entry, the card holders name can be changed in the credit card information field.
