Athlete Waivers
All participants of a USBC national bowling event are required to submit an Acknowledgment of Assumption of Risk and Waiver of Liability form. This form needs to be completed once for the 2025-2026 bowling season.
Any athlete who will be 18 years of age or older as of July 18, 2026, is required to complete the Adult Athlete waiver. If you have any questions or concerns about the athlete waiver, please EMAIL WAIVERS.Other Tournaments

Survivor Tournament
- Rules
- Participants are limited to one entry into the Survivor Tournament
- Limit one (1) bowling ball per entry
- If registering multiple bowlers, you will need to do each one in a seperate transaction.
